Cindy Kowalski, Licensed Medicare Advisor, Illinois
Your Illinois Medicare Advisor

Hi, I’m Cindy Kowalski

I’m a licensed, independent Medicare advisor serving clients across Illinois. I hold BCBS of Illinois/HCSC certification and am appointed with major carriers competing in the Illinois market. I got into this work because I almost made the wrong Medicare decision myself when I turned 65 — and I wanted to give other people the honest guidance I wished I’d had.

I’m not tied to any carrier. I compare every option available in your Illinois county — including BCBS IL, Humana, Aetna, UnitedHealthcare, and others — and tell you exactly what each one means for your doctors, your medications, and your budget.

What Makes Medicare in Illinois Different

Illinois has a large and competitive Medicare market — particularly in the Chicago metro — but it lacks some of the Medigap protections that neighboring states like Indiana now have. Illinois has no Birthday Rule, which means after your initial 6-month guaranteed issue window at 65, switching from one Supplement plan to another typically requires medical underwriting.

This makes Illinois one of the states where getting it right the first time matters most. The carrier you choose at 65, and the plan type you choose, may be harder to change later than you think.

The Chicago metro is also one of the most aggressively marketed Medicare regions in the country. During AEP, television, mailers, and phone calls from carriers can make it nearly impossible to know what’s actually good vs. what’s just well-advertised. An independent advisor cuts through all of it.

🏠 Moving from Indiana to Illinois (or vice versa)?

Cindy is licensed in both Illinois and Indiana. If you’re relocating across the state line — or splitting time between the two states — she can help you navigate the transition without gaps in coverage or penalty exposure. Moving triggers a Special Enrollment Period, and the plan that worked in Indiana may not be the right plan for your new Illinois ZIP code.

What Illinois Doesn’t Have

Illinois Has No Birthday Rule —
Your First Decision Carries the Most Weight

🎂

No Birthday Rule in Illinois

Unlike Indiana (2026) and Florida, Illinois does not give Medigap enrollees an annual window to switch supplement carriers without underwriting. After your initial 6-month window at 65, changing plans requires health qualification in most cases.

🔒

Underwriting After 65

If you choose Medicare Advantage at 65 and later want to switch to a Supplement, Illinois carriers can deny you or charge higher premiums based on your health history. The window at 65 is the one time you’re guaranteed acceptance.

📋

Strong Initial Protections

At 65, you do have a federal 6-month guaranteed issue window. During this time you can enroll in any Medigap plan with no health questions asked. This is the window to understand and use well.

⚠️ Why This Matters in Illinois

Because Illinois lacks the annual switching flexibility that Indiana and Florida offer, the plan you choose at 65 — Advantage vs. Supplement — is harder to reverse. An independent advisor who shows you both options side by side before you enroll is more valuable in Illinois than almost anywhere else.

Your County

Medicare in Illinois Varies by County

The right plan in Cook County may not be available in DuPage or Kane. Here’s what makes each area different.

🏙️ Cook County (Chicago)

Cook County has the most plan competition in Illinois — BCBS IL, Humana, Aetna, UnitedHealthcare, and others all compete here. Major health systems include Northwestern Medicine, Rush, Advocate Health, and University of Chicago Medicine. Network verification is critical: not every plan includes every major system, and some plans that cover Northwestern don’t cover Rush and vice versa.

🏠 DuPage County (Naperville, Wheaton)

DuPage has strong plan options but a different mix than Cook County. Edward-Elmhurst Health and Advocate are major systems here. Some plans available in Cook aren’t available in DuPage, and the premium structure can differ significantly even with the same carrier. Always compare at your specific ZIP code.

🏠 Lake County IL (Waukegan, Libertyville)

Lake County IL sits on the Wisconsin border with its own plan availability. Advocate Condell and Northwestern Lake Forest Hospital are dominant here. Some Wisconsin-based carriers also have IL Lake County network options worth reviewing.

🏠 Will & Kane Counties (Joliet, Aurora)

Will and Kane counties are among the fastest-growing in Illinois, with plans that reflect both suburban Chicago access and more rural network characteristics in outer areas. Residents in the outer collar counties should verify that their preferred providers are truly in-network — not just nominally listed.

📍 Downstate Illinois — Cindy Can Help Too

Whether you’re in Rockford, Peoria, Springfield, Champaign, or a rural downstate county, Cindy compares what’s actually available at your specific ZIP code. Plan availability downstate can be thinner than in the metro, which makes independent guidance even more important.

Know Your Dates

Your Illinois Medicare Enrollment Timeline

You have a 7-month window around your 65th birthday. Miss it and you face permanent Part B penalties — added to your premium every month for the rest of your life.

3 Months Before

Your Window Opens

The ideal time to start comparing plans. Cindy can pull every plan available in your Illinois county and walk you through what each one means.

Birthday Month

You’re Eligible

Parts A and B activate. If you haven’t enrolled yet, you’re still in the window — but don’t wait. Coverage gaps create billing complications.

3 Months After

⚠ Window Closes

Miss this and you face a 10% Part B penalty permanently added for every 12-month period you were eligible but didn’t enroll.

📅 Still Working Past 65 in Illinois?

Illinois has a large professional workforce — healthcare, finance, law, and government — that often works past 65. If you have creditable employer coverage (20+ employees), you may delay Part B without penalties. If your employer has fewer than 20 employees, Medicare becomes primary at 65. Cindy will tell you exactly which situation applies to you.

Not sure when you should enroll? We’ll create your personalized Illinois enrollment plan.

Avoid These Pitfalls

The Most Common Medicare Mistakes Illinois Residents Make at 65

⚠️

Missing the Part B Enrollment Deadline

A 10% penalty permanently added for every 12-month period you were eligible but didn’t enroll. This follows you for life regardless of which plan you eventually choose.

🎂

Assuming Illinois Has a Birthday Rule

Neighbors in Indiana now have a 60-day annual window to switch Supplement carriers. Illinois does not. After your first 6 months on Part B, switching Medigap carriers requires underwriting. Don’t assume the flexibility exists.

🏥

Not Verifying Your Doctors Are In-Network

Northwestern, Rush, Advocate, and University of Chicago Medicine are not all in-network for every plan. Your specialist may be out-of-network even on a plan that sounds like it covers the Chicago system you use.

🔒

Choosing Advantage Without Seeing the Supplement Side

Illinois agents who only sell Advantage will only show you Advantage. An independent advisor shows you both. In a state without a Birthday Rule, this comparison before you enroll is more important than anywhere else.

💲

Choosing Based on $0 Premium

Many Illinois Advantage plans advertise $0 premiums. But a $0 premium plan with a $6,700+ out-of-pocket maximum isn’t free. Cindy shows you the full cost picture for your specific situation.

🗺

Using Cook County Data for a Collar County

Statewide and Cook County averages don’t apply to DuPage, Lake, Will, or Kane. Plan options, premiums, and networks differ by county. Always verify at your actual ZIP code before making a decision.

These mistakes can follow you for decades. Let’s make sure you avoid them.

Common Questions

Illinois Medicare FAQ

These are the questions we hear most from people turning 65 in Illinois. If yours isn’t here, call or request a callback — no question is too basic.

You have a 7-month Initial Enrollment Period — 3 months before your 65th birthday month, your birthday month itself, and 3 months after. Enrolling in the first 3 months ensures Day 1 coverage. Enrolling late without qualifying employer coverage results in a permanent 10% Part B penalty for every 12-month period you were eligible but did not enroll.
No. Illinois does not have a Birthday Rule like Florida or Indiana. After your initial 6-month guaranteed issue window when you first enroll in Medicare Part B at 65, switching from one Medigap plan to another in Illinois generally requires medical underwriting. This makes your first enrollment decision at 65 especially important in Illinois.
Yes significantly. Cook County, DuPage County, Lake County, Will County, and Kane County each have different plan availability, networks, and pricing. Some plans available in Cook County are not available in collar counties, and vice versa. Always compare plans at your specific ZIP code rather than relying on general Chicago metro information.
Blue Cross Blue Shield of Illinois (HCSC) is one of the dominant Medicare Advantage carriers in the Chicago metro area. BCBS IL plans often include Northwestern Medicine, Rush, and Advocate Health in their networks, but coverage varies by plan type and county. An independent advisor compares BCBS IL alongside all other carriers available at your ZIP code to find the best fit for your doctors and budget.
No. COBRA does not count as creditable coverage for Medicare Part B purposes and does not extend your Special Enrollment Period. Delaying Part B while on COBRA can result in permanent late-enrollment penalties in Illinois as in every other state.
No. Independent Medicare advisors are compensated by insurance carriers when a client enrolls. Your premium is exactly the same whether you work with an advisor or enroll directly. There is no fee to you. Cindy Kowalski at Eligry compares every plan available in your Illinois county with no carrier loyalty and no cost to you.
